To refer a patient to Pikes Peak Hospice & Palliative Care, please call our Admissions/Access Services professionals at (719) 633-3400 or fax us at                        (719) 457-8101. 

Please include:

  • Patient Name

  • DOB

  • Diagnosis
  • Reason for Referral

  • Contact Information

We are happy to meet with the patient and their family to provide information and assess how we can best meet their needs.
