Where will I receive care?

Care is provided where patients and their families are most comfortable:  in their own homes, in a long term care or assisted living facility, or when necessary, in our own acute care Inpatient Unit


Hospice Care at Home

More than half of our patients receive care in their own homes, where they can be surrounded by family and friends, and supported by a team of hospice professionals.  In addition to providing physical care as needed, and emotional and spiritual support for both patient and family, we also help caregivers with all the equipment, training and support they'll need.


Long Term Care and Assisted Living Facilities

For other patients, a long term care or assisted living facility has become their home and we bring full hospice care to them there.  Working alongside the facility's staff and the patient's regular physician, we collaborate to develop a personal plan of care.  Facility staff continue to provide care, meals, supervision and support, and social activities.  Our team works to supplement that care with specialized services aimed to add comfort, symptom management and quality of life.


Peak Hospice Inpatient Unit

When the patient's needs are too complex for care in another setting, we offer comprehensive hospice care in our licensed hospice Inpatient Unit with comfortable, home-like surroundings.  Visiting hours are open, and family, children and pets are welcome.

Patients can also check into the Inpatient Unit for short-term respite care, or to stabilize pain and symptoms.
