Care Synergy Affiliation

Pikes Peak Hospice & Palliative Care entered into an exciting new partnership June 2016. We became an affiliate of Care Synergy, a not-for-profit organization created to support, sustain, emplower and grow mission driven, values based not-for-profit hospice, home health and palliative care organizations providing community based care and services. Other affiliates include The Denver Hospice (serving the nine-county metro-Denver area), Halcyon Hospice (serving the northern Front Range to Castle Rock), Pathways Hospice (serving Larimer and Weld Counties) and most recently, Colorado Visiting Nurse Association.

Under the Care Synergy operating model, each of the five affiliates operate as distinct and independent not-for-profit providers with their own local governance.

Pikes Peak Hospice Foundation is a separate 501(c)(3) tax-exempt corporation. All gifts given to Pikes Peak Hospice Foundation will remain in our community, and will continue to fortify the opportunities and growth of Pikes Peak Hospice & Palliative Care's programs of care.

We are honored to be one of the fine organizations comprising this innovative affiliation model, and look forward to expanding excellence in end-of-life care with exceptional providers along Colorado's Front Range.
